Have a real conversation. Do your homework before the discussion. I had a 99 yr old patient who was 87 lbs. Full code was the families choice. I empathized with them, but also spoke in truth when explaining if he were to need chest compressions, he would possibly end up with broken/contused ribs r/t his fragility. His outcome could possibly be more painful and prolonged due to his family wanting to hold on the this cherished man. It's ultimately the patient/
Family's choice. Be sure to educate so they can make the best decision possible for their loved one.

First you find out the patients written wishes and discuss from that point. Legality is everything and frequently family members fight over what is right.
