Absolutely, Case Management is probably the best option, there are both remote and hybrid options. I have been case managing patients for Internal Medicine for over 12 years. These are medically complex patients that need support managing their conditions when at home. If you love to teach and develop actual relationships with patients, this may well be a job for you.
