What is the best advice for a home health nurse looking to transition to clinic/hospital setting?
Since you have worked with a lot of patients in the home with chronic medical issues, a MED/SURG unit for a year would be your best transition or even the ER. Good Luck!
It will be fast paced the hospital eats their young so be prepared

Be sure to familiarize yourself with a computer system they use. Find out what the nurse patient ratio is and be sure you're comfortable with that. Of course this depends on what type unit you're working on. Kind of hard to go from home health to a clinic hospital setting for me. But it worked well make sure you get thorough orientation till you feel comfortable with where and how you'll be working.
Start with the population you serve in home health (peds, hospice, geriatrics, etc.). It will help the employer know you already have experience and can help their population more. I'd recommend clinic over hospital, easier transition.
One recommendation is for the home health nurse to get acquainted with new schedules that may not be as flexible as the home health role. The pace of work can be accelerated if working in a large active practice or hospital. Ask what the ratio is for nurse to patient to begin to visualize how the day- day may be. Also consider shadowing a nurse for 3-8 hrs on their busy days- this will give you a window in as well and will hopefully yield a smoother transition.
Best advice would be to avoid hospital but look for ambulatory position for better hours and no on-call.

It is all about asking if you could follow someone for for a short time. Ask if you could have a preceptor for a few weeks.
