What are some common mistakes new scrub nurses make with surgical instruments, and how can I avoid them?

Always ensure that you have properly counted and have recorded (with the circlulating nurse) the instruments and surgical sponges. Do not let the surgeon close the patient until this is completed. This is taught to all new OR nurses and Techs, but it is incredibly important. I once had a surgeon refuse to listen to me regarding a missing instrument. I called for an x-ray. I was correct. The surgeon was thankful that we did our job correctly. Do not let someone intimidate you if you know you doing the best thing for the patient. Remember that your name is on that patients medical record. You are an important part of their experience. It is much easier to remove the retained item prior to closing the patient!!
